Can my regular doctor perform the medical examination?
No. Only USCIS-approved physicians familiar with immigration medical policy and updates can perform the I‑693 exam in the U.S. (Civil Surgeon). Outside the U.S., the exam is performed by a Panel Physician.
Which form needs to be submitted for the immigration medical exam?
If applying in the United States, you submit Form I‑693 completed by a designated Civil Surgeon. If applying outside the United States, you may use Form DS‑2053 through a U.S. consular process. We’ll guide you to the correct form requirements.
What documents are required for the medical exam?
Bring your passport, driver’s license, or state ID. Nice to have: vaccination records and your Alien Registration Number (A‑Number).
What happens after the medical exam?
After completion, the Civil Surgeon certifies the results on the appropriate forms and places them in a sealed envelope labeled “DO NOT OPEN THE SEALED ENVELOPE.” USCIS can reject forms if the envelope is opened or altered. You (the applicant) submit the sealed envelope to USCIS with your application.
How long is my medical exam valid?
The form must be signed by a Civil Surgeon no more than 60 days before filing the underlying immigration application and remains valid for two years following the date signed. Schedule close to your filing date while allowing time for lab testing.
